SMOD: Add option for a reason behind a kick/ban

It would be great being able to add a reason for a kick/ban, which displays in the disconnect message.

smod kick/ban <ID> <reason> - Since kick can be used as a warning measure, would be great if one could add a reason behind it.
smod tempban <ID> <length> <reason>

And yes, sure, one can just simply warn them in the chat, but not many people care to even read it/ignore it. With a kick message, it would be hard to ignore.

Additionally, adding a reason behind your actions is a feature almost every moderation tool has. And would be very useful for logging purposes, too.

Adding a reason shown for the initial kick with kicks/bans/tempbans would be a relatively quick change. Anything persistent would require saving the reason for the ban with each individual ban entry which is a fair bit more to consider.
